Dr. Stanley Jack
Dr. Stanley Jack has been in private practice since 2007. After receiving a B.S. in Biology and a B.A. in Psychology at the University of Colorado at Denver, Dr. Jack attended medical school at Des Moines University College of Osteopathic Medicine and Surgery, during which he did a year of clerkships in the Cleveland Clinic Health System. He then went on to complete an internship in Internal Medicine and his residency in Family Medicine, both at Exempla Saint Joseph Hospital in Denver, Colorado.
Prior to private practice, Dr. Jack provided care in the ambulatory setting as a Family Physician and an Urgent Care Physician. Other areas of medical interest include Sports Medicine, Osteopathic Manipulative Medicine (OMM), and Wilderness Medicine. Dr. Jack is an Associate Clinical Professor for the Rocky Vista University College of Osteopathic Medicine, and is a Basic Life Support and Advanced Trauma Life Support Instructor.

Ciani Kate Sosa
Ciani is a Family Nurse Practitioner with well over a decade of medical experience. She has dedicated herself to Family Medicine ever since graduating in the top of her class from Regis University in 2019. Prior to that she provided care for patients in a variety of settings, ranging from obstetrics to oncology after graduating with honors from the University of Northern Colorado Nursing Program in 2011.
Ciani has a heart for the underserved, and strives to provide excellent care for all of her patients. She believes in being proactive about her patients’ well-being, and encourages health and disease prevention early on with an emphasis on behavioral modification through diet and exercise. Ciani is proficient in both English and Spanish, and enjoys helping others on medical missions trips.

Natalie Ibrahim
Natalie Ibrahim completed a Master’s degree in Physician Assistant Studies at Christian Brothers University. She received her B.S. in Biology from the University of Memphis in her hometown of Memphis, Tennessee. During college she worked as a Hospital Pharmacy Technician, and after her graduation she did research for a few years at St. Jude Children’s Research Hospital. In 2017 she and her husband moved to Colorado where she began her career in Family Medicine. She joined Boulder Valley Family Practice in 2021.
Natalie has a special interest in preventative care and enjoys empowering patients to improve their overall quality of life through education and solutions that fit their unique lifestyles. In her free time, she loves enjoying the outdoors with her husband and twin boys.

Darrell Creekmore
Darrell Creekmore attended California State University Stanislaus as an undergraduate where he earned a Bachelor of Science in Biology and Chemistry, graduating with honors. He became a Physician Assistant in 2017 after earning a Master’s degree at Samuel Merritt University in Oakland, California. Prior to Family Medicine, Darrell worked in Emergency Medicine and Urgent Care.
Darrell practices personalized healthcare and believes the quest
for optimal health begins with treating the human body as a whole. He employs
evidence-based medicine but with a natural approach to healing and the
utilization of supplementation as an alternative to pharmaceuticals when
appropriate. Darrell holds a special interest in sports medicine,
nutrition, human optimization, longevity and functional medicine. When Darrell
is not seeing patients, he enjoys hiking, High-Intensity Inverval Training (HIIT), exercise, yoga and cooking
for his wife.
